what are dermal fillers?
Dermal fillers are game changers in the field of medical aesthetics. For the last decade the choices of fillers and advancements in techniques have allowed us to reverse the effects of aging, perfect and celebrate our natural beauty, and to add finesse to our facial features. Dermal fillers made with Hyaluronic acid (HA) are designed to replace volume loss in the areas of cheeks, temple, mid and lower face, chin, jawline and lips. Hyaluronic acid is a polysaccharide (glucose molecule) which is naturally occurring in our body as it is a building block of our connective tissues.
how do dermal fillers work?
We are fortunate to offer many HA fillers in our practice. However, not all fillers are the same and are therefore not interchangeable. Fillers have unique chemical and physical properties, and are designed to be injected into specific areas of the face. Fillers that are intended to create a lift are thicker gels, which create vertical lift. Other fillers are designed for mobile areas, such as lips. Those fillers have unique properties allowing for free and unrestricted mobility of the tissues. Fillers have different longevity based on their physical structure, mobility of the treatment area, and metabolic rate of the client. For example, fillers placed in lips have the shortest duration due to constant movements of the lips (3 to 9 months). Fillers used for cheek bone enhancement have the most longevity, up to 12-24 months.
am i a good candidate for dermal filler?
This is a great question. Filler is indicated to treat volume loss of the face, for correction of asymmetry, and for diminishing fine lines and folds. Clients often wonder if there is a particular age they should be starting to think about fillers. The answer is no. We frequently treat younger clients with the purpose of beautification, such as hydrating and defining lips, chin and jawline contouring, and correction of asymmetry. Restoration treatments are applicable for more mature clients who frequently look for replenishing of volume loss, correction of folds and fine lines.
There are several contraindications for fillers based on one’s medical conditions. Some of the contraindications include history of severe allergies, pregnancy and breastfeeding, known allergy to HA filler or lidocaine, active infectious process, and being treated with blood thinners. As far as aesthetic indications, we make those determinations on a case by case basis.
whAT CAN I EXPECT DURING DERMAL FILLER TREATMENT?
Your provider will discuss with you the procedure, including goals, realistic expectations, potential complications and after care. In our practice we utilize ultrasound to avoid inadvertent arterial injection, and we use Accuvein device to avoid bruising. We also use micro cannula technique to ensure safety and accuracy, and to minimize any chance of bruising. Your comfort during this treatment is our great priority. We use various techniques to enhance your relaxation and minimize discomfort.
HA fillers are reversible by a naturally occurring enzyme called hyaluronidase which can be used to reverse HA filler in the rare case of complications of undesirable effects.
HOW DO i PREPARE FOR DERMAL FILLER BEFORE MY APPOINTMENT?
Unlike many other aesthetic procedures, filler requires very little preparation. In order to minimize bruising, we recommend avoiding alcohol, aspirin, ibuprofen-type medications, flaxseed and vitamin E 2-3 days prior to treatment. Please let us know if you are recovering from infections, have been recently vaccinated or scheduled for vaccination in the next 2 weeks, and if you EVER had a history of cold sores.

WHAT IS A NEUROMODULATOR (BOTOX, DYSPORT)?
Botox is the “Kleenex” of neuromodulators. In other words, when we think of Botox, some of us don’t realize there are more than one of its kind. Neuromodulators are the family of onabotulinum toxin A proteins, which temporarily relax muscles. Botox, Dysport, Xeomin and Jeuveau are four neuromodulators currently approved by the FDA in the United States.
When making facial expressions, lines form on your face as muscle under the skin contract. Each smile, frown, or squint you make contributes to the wrinkles that appear on your face. By temporarily relaxing muscles, neuromodulators soften facial creases and undesirable lines that form over time. Left untreated, some of these facial lines may leave you with unintentional worried or angry expressions.
what is the difference between botox & dysport?
Both Botox and Dysport contain Botulinum toxin Type A (BoNT-A). Botox is manufactured by Allergan, while Dysport is manufactured by Galderma. All BoNT-A products have the same size and weight of active neurotoxin. However, Botox and Dysport have different accessory protein complex components. Dysport is known to have a faster onset of action. 30% of patients have some response to Dysport within 24 hours, 73.3% have response within 48 hours, and 86.7% have response within 72 hours. However, the full effect of either Dysport or Botox will be apparent after 2 full weeks.
For most people, Botox and Dysport lasts for about 3 months. For many patients, Dysport also has a longer duration of action compared with Botox, sometimes up to 4-5 months. Results will vary depending on your individual muscle size, strength, and activity, among other factors.
WHAT ELSE CAN botox or dysport TREAT BESIDES WRINKLES?
Gummy smile - showing too much gums when smiling can be alleviated by relaxing the muscles that elevate when you smile.
TMJ - teeth grinding or clenching causing jaw soreness can be improved by relaxing the masseter muscles that help with chewing.
Uneven facial expressions - stronger muscles causing uneven facial expressions can be relaxed to create symmetry.
Facial contouring - muscles tend to atrophy (shrink) after repeat dosing of neuromodulators. Thus jawline slimming can be achieved after treating the masseter muscles that help control chewing.
Acne - by reducing pore size and sebum (oil) production, there is improvement of acne breakouts.
Excessive sweating - by relaxing the sweat glands, there is less sweat production.
Hair restoration - by relaxing the ciliary muscles that are attached to hair follicles, there is improved blood flow. Improved blood flow to the scalp allows for better nutrient (growth factor) supply to the hair follicles. One study showed 8 of 10 people treated with Botox for male pattern baldness had good to excellent results: "A Pilot Study to Evaluate Effectiveness of Botulinum Toxin in Treatment of Androgenetic Alopecia in Males"
WHAT TO EXPECT with botox or dysport treatment?
Reducing the use of facial muscles with a neuromodulator can help smooth the appearance of facial expression lines for 3 to 4 months. Duration varies between individuals and may be affected by your metabolic rate.
During consultation, your provider at Rejuve Medspa will discuss your medical history, observe animation of your facial muscles, and develop your treatment plan. After marking targeted muscles with brow pencil, you will be injected with Botox or Dysport using a very small gauge needle. Due to the proper technique and fine size of the needle, the discomfort is minimal. You may have slight redness at the injection site, which disappears in less than an hour after treatment.
IS THIS botox or dysport RIGHT FOR ME?
Botox Cosmetic is FDA approved for adults for temporality improvement in the appearance of moderate to severe frown lines, forehead and lines around the eyes known as crow’s feet. Off label use includes correction of “gummy smile”, lip lines known as “smoker’s lines”, wrinkling in the area of chin, and injection in masseter (muscle involved in chewing) for the purposes of slimming effect as well as relief of clenching and teeth grinding.
This treatment is not recommended for individuals who have infection at the proposed injection site, during pregnancy or lactation, anyone with known hypersensitivity, or in presence of certain neurological diseases.
IS THERE ANY DOWNTIME with botox or dysport?
Besides the possibility of mild redness at the injection site lasting less than an hour, this procedure is considered to have no downtime. We recommend abstaining from massaging the area, lying down flat and avoiding vigorous physical activity for about 4 hours after the treatment to prevent migration of the product from strategically injected areas.
WHEN CAN I EXPECT RESULTS with botox or dysport?
While the effects of an injection are not immediately apparent, results are visible after a few days. However, the full effect takes 14 days before you appreciate the final outcome.
WHEN SHOULD I FOLLOW UP?
FDA treatment recommended doses come in ranges. If you are a new client, we will start you on the lower end of the therapeutic range and will reassess in two weeks if you would require additional Botox enhancement. Once you are pleased with your outcome, no follow up is needed. However, we will be delighted to meet with you for any questions or concerns.
